Document how to use realm.properties and ldap configuration

Document a configuration that shows how to use both a realm.properties (that may contain 0 or more "emergency" logins and groups) along with a connection to an LDAP directory (eg ActiveDirectory)

See attached files for an example configuration. The changes are summarized in the attached diff.
Note: the realm.properties should contain the prefixed rolenames (role.rundeck.admin, etc.) according to on-site policy.

Two outstanding problems with this configuration:
1) If the user is not in the LDAP, but is in the fallback, there is an NPE thrown but it's harmless.
2) There seems to be some internal authentication that's not succeeding and causing login hits. Still investigating. -
